Host Home Providers create a caring support system and assist the individual with personal care, meals, finances, medical appointments and community connections. The people who receive services in a Host Home Provider’s home have varying degrees of developmental disabilities such as autism, cerebral palsy or epilepsy. Some may be very independent and able to go out into the community on their own and even have a job, while others will need more support. Our staff will match you with a person whose needs fit your lifestyle. Host Home Providers are independent contractors, not employees, with Community Living Alternatives, Inc. . As a Host Home Provider, you are required to provide supervision and training to the person in your home.
Recommendation of approval from the Colorado Department of Public Health and Environment to provide Specialty 676 Respite CES/SLS and a Home Care Agency Class A or Class B license – if applicable. Massage Therapy providers shall be in good standing with the Colorado Office of Massage Therapy Licensure. Malpractice/Liability insurance information must be entered on the application.

Our management team averages 15 years of experience in the field of Developmental Disabilities. Some CPT procedure codes are grouped with other related CPT procedure codes. Host homes come in all shapes in sizes depending on what the person is looking for in a living companion. Overture has host homes throughout the state of Colorado in various cities, suburbs, and more rural areas. Home means something different to everyone and some of our host homes have families that the person can live with, other host homes may be just two people living together. We support host homes with 1-2 people receiving services living in the home with a designated Host Home Provider. On some occasions, if it is the persons' choice to live with three people receiving services, Overture and the Case Management agency may work with that host home to get a three person waiver to allow for this living arrangement.

The authorizing agent or case management agency/CCB is responsible for timely submission and distribution of copies of approvals to agencies and providers contracted to provide services. Approval of prior authorization does not guarantee Health First Colorado payment and does not serve as a timely filing waiver. Prior authorization only assures that the approved service is determined to be necessary and is considered a benefit of the Health First Colorado. Electronic Visit Verification is required for some services billed under this specialty code and will be automatically added with an approved application for Health First Colorado (Colorado's Medicaid program).

A resume that demonstrates at least one year of experience in the provision of music therapy to pediatric/adolescent clients for each individual providing services. Nurturing and supportive Host Home providers offer a healthy and fulfilling family living environment for individuals with disabilities.
A Host Home Provider may contract to care for up to two individuals in their home. Laradon thoroughly screens all Host Home Providers and conducts background checks. In addition, Laradon staff provides Host Home Providers with ongoing training, guidance and support.

You see, our mission is much more than simply providing cost-effective care and innovative services. Every situation is different, yet all Home Host Providers act as thoughtful advocates for an individual in their care. Depending on the level of attention required, some Home Host Providers work full- or part-time, while the person in their care is attending a day program.
Applying with Overture does not guarantee placement. Continuum of Colorado is a 501, nonprofit agency providing services to people with a wide range of abilities. Although we’d love to work with everyone, we evaluate potential providers best positioned to care for a client.

A “Host Home” is a residential program model providing people with developmental disabilities a stable home environment. The program is designed to allow a person to live with a family or individual without disabilities. The focus of this model is to get away from constantly changing staff situations. In essence, the person becomes a member of the family.
