Host Homes serve up to 2 people in the home, with an option to provide temporary/backup service for a 3rd if there is another available bedroom in your home. Our management team averages 15 years of experience in the field of Developmental Disabilities. Some CPT procedure codes are grouped with other related CPT procedure codes. When more than one procedure from the same group is billed, special multiple pricing rules apply. Case management agencies/CCBs complete the Prior Approval and/or Cost Containment requests for their specific programs according to instructions published in the regulations for the Department of Health Care Policy and Financing . The authorizing agent or case management agency/CCB is responsible for timely submission and distribution of copies of approvals to agencies and providers contracted to provide services. Approval of prior authorization does not guarantee Health First Colorado payment and does not serve as a timely filing waiver. Prior authorization only assures that the approved service is determined to be necessary and is considered a benefit of the Health First Colorado. Electronic Visit Verification is required for some services billed under this specialty code and will be automatically added with an approved application for Health First Colorado (Colorado's Medicaid program).

Providers submitting new HCBS enrollment applications must watch the "Enrolling as a Health First Colorado Home and Community Based Provider" Training and successfully complete the "Provider Enrollment Quiz." When selecting multiple specialties, select all appropriate specialties/services on the same application. Possess good verbal and written communication skills to communicate with CLA employees, Case Managers, parents or guardians and professionals involved in the individual’s life. We will contact the Host Home provider after a match has been made. Our placement team will set up opportunities for both parties to meet and get to know a little bit about each other.

Thus, we coordinate regular social gatherings for our host home providers, clients and families to provide the opportunity to get to know one another, and establish partnerships, mentorships and friendships. A separate component of TCM encompasses face-to-face monitoring the quality of services to ensure the member is receiving services in accordance with the service plan. What Does a Host Home Provider Do?
CCBs don't have one 'Provider Type' or 'Specialty' assigned to them that encompasses everything they do. CCBs will need to complete a couple different applications with us, depending on the services they provide. Support, Inc. ensures that all Host Home Providers and adequately trained and skilled in areas of general care, Medication Certification, First Aid and CPR, positive behavior supports, Consumer Rights, etc.

To redefine the experience of community living for people with intellectual and developmental disabilities to the point of there being no question as to where individuals and families choose to be served. Host Home Providers create a caring support system; assist the individual with personal care, meals, medical appointments, and community involvement through engaging activities. Laradon does an excellent job of matching each adult to the provider’s lifestyle. Most people who choose to live with a family will also have a job, work training or other activity during the day. However, employment or other day activities are not required and some people may wish to stay home during the day. Our team will work with you to be sure that the needs, preferences and choices of the individual who comes to live with you can be supported by your schedule.
Must submit applicable licensure for Program Director to include Licensed Psychologist with demonstrated applicable experience, or LCSW, PT, OT, SLP, RN or Masters level Psychologist with demonstrated applicable experience. Must submit a copy of the license along with a resume, transcript and/or curriculum vitae for review. In-state presence required for out-of-state providers for installation and service. DDRC is seeking Host Home Providers in the Metro Denver area to open their home to a person with an intellectual / developmental disability, and to provide that person with responsible, caring help, appropriate for their needs.

You only need to submit additional applications if you have additional service locations or if you are applying for an additional provider type outside of HCBS. Host Home Providers may work with up to two individuals with disabilities in their home, depending on the number of bedrooms/bathrooms and the amount of shared space that is available. Individuals are able to choose their roommates, and a 30-day trial period is required to ensure the Host Home is an appropriate placement for the individual if agreed by entire team.

Foothills Gateway currently has several host homes in Loveland, Fort Collins, Greeley, Windsor, and Wellington. Some of the host homes have one individual and others have two. The rates are based on what we receive from the state of Colorado, based on each individual person’s Supports Intensity Scale .

Carry full insurance coverage on your automobile, which meets the minimum state liability requirements and full home/renter’s insurance which is adequate to cover the possessions of the individual living with you. Be eligible to work in the developmental disability system in which you would be approved by any Community Centered Board . When selecting a family, we consider ethnicity, religion, marital status, children in the home, employment status, financial stability, compatible personalities and openness to ongoing contact with family.
